Physician referral · Dental sleep medicine · New Jersey
A referral pathway for CPAP-intolerant OSA patients — with objective multi-night data returned to you on every case.
You diagnose and prescribe. We fit and manage a custom, titratable oral appliance under the joint AASM/AADSM guideline — and send you a before/after multi-night report (including hypoxic burden).
Seen within 5 business days of patient contact · Same-week confirmation to your office · FDA-cleared SleepImage ring (K182618) · Fee quoted to patient before any treatment
For sleep physicians, cardiology, primary care & ENT — not a patient intake page.
The problem we solve for your practice
Every sleep practice carries patients who declined, abandoned, or cannot tolerate PAP. They remain symptomatic. Airway.Dental is built for that panel: oral appliance therapy with dental-side surveillance and data you can act on.
Who to refer
• Diagnosed OSA + CPAP-intolerant or non-adherent
• Mild-to-moderate OSA who prefer OAT per your prescription
• Patients needing fitting, titration, and dental-side follow-up of a prescribed appliance
• Resistant-hypertension + diagnosed OSA off CPAP — we return nightly hypoxic-burden trends alongside the clinical picture you already manage
Explicit boundary: We do not diagnose sleep apnea. If the patient is not yet diagnosed, order a study first; then refer for appliance therapy and multi-night monitoring.
How co-management works
1. You prescribe — diagnosis and treatment selection remain yours.
2. We aim to see them within 5 business days of patient contact — Morristown or Metuchen, with same-week confirmation to your office by fax or email.
3. Custom titratable appliance — adjusted in small increments.
4. Objective verification — multi-night home recording with SleepImage (K182618).
5. Data back to you — plain-language before/after report: breathing disturbance, sleep quality, fragmentation, hypoxic burden. Patient returns to your care; their dental home is unchanged unless you prefer otherwise.
Why multi-night data
Night-to-night AHI variability is well documented. Single-night snapshots can mislead titration. We track 10–20 nights so you see a stable treatment response — including hypoxic burden, increasingly tied to cardiovascular outcomes.
What you receive on your patients — de-identified sample of a 23-night VTP™ report (shared with permission).

Download the fillable referral form and fax it with your Rx/order for oral appliance therapy and the sleep study. A physician-ordered sleep study is required before referral. We aim to see referred patients within 5 business days of patient contact, with same-week confirmation to your office.
Scan for the referral form, or type airway.dental/referral-form. Fill it in on screen, save, and fax it with the sleep study.
