You may have heard your sleep apnea called “mild.” But one number — your AHI score — doesn’t tell the whole story. A newer measurement called hypoxic burden looks at what those breathing pauses are actually doing to your body.
The short version
- Your AHI score counts how many times your breathing pauses at night — but not how serious each pause is.
- Hypoxic burden (HB) measures how often, how far, and how long your oxygen drops while you sleep.
- Research shows HB can reveal health risks that an AHI score alone would miss — even when the AHI looks “mild.”
What is hypoxic burden?
When your airway narrows during sleep, your oxygen level dips. Hypoxic burden adds up three things across the whole night: how often your oxygen dips, how low it goes each time, and how long it stays low. Think of it as the total toll those pauses take on your body — not just a tally of how many happened.
Why isn’t my AHI score enough?
The AHI has been used for decades, but it treats every breathing pause the same. A brief, mild pause and a long, severe one each count as “one event” — even though they affect your body very differently. Short events that still drop your oxygen can be missed entirely.
That means two people with the same AHI score can carry very different risks. Here’s a real example — three patients whose AHI scores look nearly identical:

On a standard test, these three look the same. But Patient 3’s body is experiencing more than twice the oxygen stress of Patient 1’s. That difference matters.
Why it matters for your health
Studies have linked higher hypoxic burden with greater risk of heart disease, heart failure, high blood pressure, stroke, and kidney disease. In long-term research, HB predicted heart failure risk more accurately than the AHI did. Notably, patients with high HB showed increased cardiovascular risk even when their AHI looked low — so a “normal” or “mild” result on a standard test may not mean you’re in the clear.
One honest caveat
Hypoxic burden is a newer measurement, and there’s no universal cutoff yet for what counts as “high.” Interpreting it takes clinical judgment. But the pattern in the research is consistent: the higher the HB, the higher the risk — which makes it a valuable part of a complete sleep evaluation.
